Once I receive the form, I will email you an invoice for the dollar amount you want to gift.
Once the invoice is paid, I will send the gift certificate to the email you indicate on the form below.

Purchase a gift certificate with Live Oak Massage* HERE. It can still be easily redeemed with me, however the recipient will also have the opportunity to book with any of the massage therapists at the practice in case my availability doesn’t work with their schedule.
